1. Please fill out the application form in English or in Traditional Chinese. 請以英文或繁體中文填妥報名資料
[IMPORTANT!] The Passport Number, Date of Birth and Email used for registration must belong to the applicant. The personal information must be valid and functional. There is no scope for changing the registered email address in future. All communications shall be sent only to the registered email address.
[重要!] 護照號碼、出生日期及電子信箱註冊資料須為申請人本人且須為有效資料，註冊後不得修改，所有資訊將透過註冊時的電子信箱聯絡。

Passport Number 護照號碼	※Please fill in your National ID Number if you don't have passport number. ※若無護照號碼，請填寫身分證字號。
Date of Birth 出生日期	年月日 year年 / month月 / date日 Enter your passport number and date of birth, then click the data verification button to retrieve your previous application form. 輸入您的護照號碼和出生日期，點選資料驗證按鈕找回您先前的申請表。
Your Password 登入密碼	Only English characters + numbers, with at least 8 characters Enter your password .(Password length: must be between 8–10 characters.) 輸入您登入密碼。
Enrolment Status 入學身分	New Student新生
Apply Degree 申請學位
